NDIS support worker invoice template

Blank, correct, free. It has the fields a self-managed participant's family (or the NDIS portal) needs: your ABN, dates of support, hours and rate, support item numbers, a "No GST" line, and how to pay you.

Or skip the typing

The online generator works out the totals, checks your ABN, prints the right GST wording and gives you a finished PDF — and remembers your details on your phone for next time. Still free, still no sign-up.

Questions people ask

Do I need an ABN to invoice a self-managed participant?
Yes — the family enters your ABN when they claim. If you don't have one, it's free from the ABR (abr.gov.au).
Do I charge GST?
Only if you're registered for it, which most independent support workers aren't. If you're not, don't add it, and say "No GST has been charged" so nobody wonders.
Do I need the participant's NDIS number on the invoice?
No. The family knows it. Include it if they ask; leave it off if they don't.
What's a support item number?
The code the NDIS uses for a kind of support (like 01_011_0107_1_1 for weekday daytime assistance with daily life). It's optional on your invoice but it helps the family claim under the right line — ask them which one they use.

Free. Without an account your invoice is made on your phone and goes nowhere else; sign in and we keep your details and invoices so you don't retype them. Made by Stray Anchor, Perth. Not tax advice: check the ATO's guidance on invoices if you're unsure.
